We take our website visitors’ privacy seriously. This policy outlines what information we collect and how it’s used.

We do not collect personally identifiable information (PII) about you unless you choose to provide that information to us. If you choose to complete an online form, know that e-mail messages can be viewed by other internet users while in transit. If you want to share confidential information with us (health concerns, diagnoses, any other private information, etc.) please call us or use traditional email. Internally, the information provided by visitors using our online forms will be shared only with authorized Mindset staff who require that information to assist the visitor or their account with us.

Mindset discloses personal information you provide only to process credit card payments or as required by law.

If you choose to opt-in to SMS messaging with Mindset, you may receive messages related to intake documents, appointments and scheduling, billing, and in response to SMS messages you send to our staff. Please note:

  • we use this data in meeting your treatment, scheduling and account needs. We do not sell or otherwise share your information or phone number outside of our organization.
  • the data is stored on secured servers with our SMS provider, RingCentral, and the provider of your phone services.
